It absolutely depends on what happened to cause that bad impression. If he just had a piece of brocoli stuck in his teeth, he'd still stand a chance, if I saw him robbing a bank then probably not.

Depends on how terrible. If it's a "put my foot in my mouth" type of thing than it's ok. But if you tried to go too far sexually or said or did something really offensive than that might be a totally turn off. Just excuse yourself or apologize and keep on with a friendly relationship, it might evolve into something more in the future.

depends on what the "terrible" impression was, if it wasn't anything too bad I think you can show that you're a good person still the next time she sees you...after all she saw you 3 other times and it seems you made a great impression, don't let one time ruin it all together.

Could be that his true personality is coming out and you are seeing what he is really like. If you should happen to see him again and you feel like it is another bad impression, i would take it as a bad sign and forget giving him a chance.

It all depends on what the third scenario is. But if all he did was make a fool of himself and embarrass himself, than for sure. Its kind of cute when guys are clumsy and try to look cool but end up looking like a dork!

that depends on what he was doing. if he was being really shady about it, then n-o. run away. if you think that he made a really bad impression on you, i say trust your instincts, but give him a chance to like explain. some times guys don't even realize what they're doing.

errrr exactly how bad was the last impression? and if it wasnt something like you found out that he cheated on your best friend's bf/gf or something really low, then yeah, he would stand a chance. everyone makes mistakes, and if i really did like him....then i wouldnt give a sh*t if he made an idiot of himself sometimes.....

if all four encounters where good ones then you would have reason to think he was pretending.. so three good ones and 1 bad one is a good sign.
