Cubed. This way you can't choke on the little pieces. Besides, it doesn't melt as fast, so it won't water down the drink.

Either / or. It doesn't make any difference to me. Maybe like Taia says in her answer, it depends on what I'm drinking, to some extent. I like the ice in my iced tea to be cubes. Cold drinks I like crushed. Margaritas I liked "on the rocks" (cubed). Just depends. God Bless you.

Small cubes, not crushed ice, not big cubes, not medium cubes but small cubes!

Cube ice is better because it don't melt as fast.
Crushed ice melts too fast specialty in alcohol.
