Usually once every 6 months or so. Can last for a week or two.
You are best to get her spayed if you aren't going to breed her, it can eliminate some health problems plus it just saves the risk of unwanted puppys and having to deal with all the blood and everything. Then she won't go into heat at all. Discuss getting her spayed with the vet if you don't want to breed her, they will give you advice on what's best to do.

I'm assuming your shepherd is a female. She will go into heat about 2x/year...starting most likely after she is 6 months of age. Heat cycles last 3 weeks in most female dogs.
It is a good idea to get your dog spayed, since it reduces the risk of cancers and other serious illness. Also, if you are not going to alter your pup, be sure to keep her away from male dogs (intact or not) during her heat cycles. This is for her own safety.

Bitches of every bred go into "heat" generally every 6 months. If you do NOT intend to use her for bredding purposes have her spayed. Spaying reduces the chances of particular cancers by 80 to 90 %. If you intend to get a litter off of her its best to let her go through at least two heat 6 cycles.

Never, if you get her spayed. You should spay her unless she is AKC registered, you plan to show her for conformation to get her championship, and get all her health and genetic tests. The world doesn't need any more puppies bred by people who aren't breeders working to improve the breed.
