Initially a heat pack helped, but now it makes me feel worse. I can't sleep from the pain, it hurts less when I dpnt move my arm and am in an upright position.

You are not taking medicines is good thing.
You ask your doctor to give pain killers and exercises for shoulder. After 15 days cut off painkillers and try only exercises. It will help.
If it fails go to acupuncturist. It is the only treatment.

I went to the er for exactly the same thing and they said it was a crick in my neck from the way i slept, but i dont think so.Im not sure what it was, but i had hubby massage it and took some anti inflammatories like ibuprofen, they seemed to help some. The doctor gave me a script for some muscle relaxers but they didnt work at all. It took a couple weeks to go away completely, and it still comes back every now and then. Maybe you will get a better diagnosis than what I got.

It could be neuralgia. See your doc to make a proper diagnosis
