if i say no to givin parental responsibility and the judge says yes do i still have to sign anything

Parental respoonsibility is nothing to worry about, it's the residency and contact orders that can create problems. ( I am speaking from experience)

You could always appeal if the order is granted but this is not really worth doing.
I have had custody of my daughter now since she was 18 months old and although my ex has parental responsibility & access orders she has never bothered with them.
My daughter has not seen her mom for 9 years now.

The ruling is of the judges and the court will only recognise this ruling wheather you agree to it or not. If you protest, get a laywer.
The judge does what is in the best intrest to the child. IF you can prove that there is a real threatning reason, like the child is in danger, or in unfit conditions, then prove it, so you can keep full responsibility.

and away from who?
but no you do not have to sign anything if you do not agree, just say no.
