He doesn't want to dress himself and can take an hour and a half to get ready in the morning. He can't peddle a bike. He wants to be picked up (he's 4) instead of walking not too far distances etc...

try a star chart and plenty of praise encourage him maybe help a bit,challenge him to race you to get dressed obviously he will win always then he could possibly get a little treat for winning(fruit or something healthy).if none of this works maybe see another DR.

Is he your only child? Has he always been like this? It sounds like he just does not want to. Try making it a race or a game. Really put your foot down, take things away. He will have to get a move on soon when school starts.

Is he overweight?? If he is that would be part of the problem. Whether he is or isn't you are going to have to start forcing him to do things & ignore the tantrums when he doesn't get his way. You are the boss in the house not him & right now he's telling you that he is in control.

Its a test & when you pass this one get ready for the next !!

you need to describe this in more detail. Usually pediatricians are not worried because children go through phases and stages all throughout their early years but until you make this question more clear i cannot give you a clear answer.

What do you mean by saying lazy? Did his/her teacher tell you that?
