since you said "little known", i would suggest alcoholic dementia and the related korsakoff syndrome, but, these are more neurologic (they might fly though, you could say they are a "mental conditon")....or how about dissociative fugue (aka psychogenic fugue) - i leave it to you to investigate what these are...it's all good stuff
[or just thumb through a dsm-iv and see if anything catches your eye]

Multiple Personality Disorder/Dissociative Identity Disorder - a psychiatric disorder characterized by having at least one "alter" personality that controls behavior. The "alters" are believed to occur spontaneously, involuntarily, and function independently of each other. Significant amnesia, which is not explained by ordinary forgetfulness, is also a recognized symptom of MPD.
There is belief that the cause of MPD is repressed memories of childhood sexual abuse.

There was something on yahoo yesterday about a type of amnesia where the person wouldn't recognize his wife after 20 seconds. I think it was the result of a brain trauma.

I wrote a paper on personal experience. I had a friend who kept having panic attacks and she didn't know it. I described her signs and symptoms, what would trigger them, etc. Got an A+. Maybe, this will work for you.
