The PORTAL system of veins drain the venous blood from the intestine and the spleen to the liver via the portal vein.
The SYSTEMIC system of venous drainage collects blood from the rest of the body to the right atrium of the heart.

There are sites in the body where the two systems join (anastomosis) and the veins get dilated when there is an obstruction to the portal vein as occiurs in alcoholic liver failure or cirrhosis. The veins in these anastomotiic sites become dilated and may bleed (porto-systemic anastomosis).

The anastomotic sites are 1.the oesophago gastric junction leading to haemorrhage.2 junction of the upper 2/3 and lower 1/3 of the anal canal leading to bleeding haemorrhoids 3. over the bare area of the liver at the back of the abdomen. 4. around the umbilicus; the dilated veins being referred to as the Caput Medusae 5. dilated veins on the sides of the abdominal wall .

The portal system of veins drains the intestines and the liver. The systemic circulation drains the rest of the body. They meet at the junction of the food pipe with the stomach, at the back of the liver and over the rectum. When the pressure in the portal veins is increased such as in chronic liver disease, the veins joining these 2 systems become enlarged and there may be bleeding from these collaterals. This can present as an enlarged liver and a large belly with water (ascites), vomiting blood and passing blood from the back passage.
