some extreme racists and such might on rare occasion object to a blood transfusion from another race. some religious aspects might have an effect. there was a case in english law where a victim died after refusing a blood transfusion on account of being a jehovah's witness.
economic factors mostly relate to the health of those willing to donate and the ability to run the necessary tests for safe transfusions in time, and the availability of blood when and where it is needed.
certain people have beliefs that loosing blood is loosing a part of the soul and may object.
all those things have an effect to varying degrees.

If the blood is screened properly then there are no problems. All The Jehovah's Witnesses should burn in hell. If my kid was dying I would sell my soul to Satan to help them. To sit by and watch them die in the name of God is messed up.
