If you're traveling by car, be sure he's got a toy and something soft to lay on. Older dogs tend to sleep more than younger ones so he shouldn't be too anxious, but ya never know. Just treat him as you do at home, be extra patient with him and he should be fine.

Where will you be staying? You've probably thought of this, but make sure the hotels, inns, etc. where you'll be staying are dog friendly. Only a small percentage are, so you don't want to be disappointed on arrival. Just do a search on pet-friendly hotels.
One of my dogs is old, but he travels in the car well. Just figure when you're ready for a bathroom break, so is he! And also for a drink of water. Bring his bowl in the car with you, and a jug of water.
Plane travel can be stressful for them; I only do it to them when I'm moving to another country (I have to travel a lot and have people watch them when I travel internationally. I only take them when I'm really moving.)
Dogs are great on travels; they generally like meeting new people, too! And they're great ice breakers. Have fun! He's lucky he gets to be with you and part of your trip.

I would not recommend flying with him unless he's been checked by a vet to make sure his heart, etc can handle the altitude (he'd have to go in cargo) If you're driving, eat a light breakfast incase the traveling is stressful on him, you don't want breakfast all over in the car. Lots of breaks to allow him to relieve himself and move around a bit. 12 dog years is pretty old so his joints might not take to long periods in the car as well as you do.

Delta is a pet friendly airplane company.If traveling by plane you can take him as carry on if hes small but if hes big u must have him in a spacious traveling crate and it will go as cargo, make sure to feed him and give him water before you departure. Inform your vet and i recommend that you dont sedate him.Check out the Delta website.

Just make sure he has plenty of pottie breaks, plenty to drink, not left in car alone for a long period for extreme heat or cold. Make sure he has plenty of room to stretch and a place to lay that is soft.. Occasional stretch stops to prevent him from getting stiff. If he's like my dogs, he will sleep most of the time.
