Tell her exactly what you're saying here, you're better off telling her, you won't spen your time wondering (What If?) she might be more than willing to give you a second chance if she still has feelings for you, and if she doesn't at least you'll know it's time to move on.

This is going to be a hard fact to accept. Your insecurities led to the breakup of the relationship and in her eyes, you are no longer the man that she "desires" since you are incapable of fullfilling the role of an alpha-decisive male. Furthermore you are in the receiving end of the dreaded "LJBF" (Lets Just Be Friends) line.
There is really only one solution. You must follow the instrucitons EXACTLY. You have to separate yourself from her for approx 3 months. Proceed to re-build yourself. Remember, when you see her in 3 months, she needs to see a WHOLE NEW YOU. Not the OLD YOU. Go work out (if your not in shape) and make yourself desirable and start dating other girls. When the 3 months is up, casually "bump" into her and bring a hot girl along with you. This will raise your social value in her eyes (since you are with a hot girl after all) and will bring you back to even-field in her mind. Also be ambigious (dont tell her you guys are dating or anything). The girl you bring along should cause a slight twinge of jealousy if you do it right. At the end, set up a date with her and tell her you want to meet alone to catch up (Seeing as to how you guys havent seen each other for 3 months). This time around, dont fook things up. There wont be a 2nd chance.

Trust me, I have been that girl. My ex boyfriend broke up with me for my bestfriend and got her pregnant. Now he wants to get back with me. She is totally over all the crap you put her threw. If you really liked her, you would have rather taken the hurting then putting her through it. No offense, not to be rude. But she is over you, I say move on.

Well, she probably is really close to not giving you any more chances, because you have messed up so many times. But if you REALLY feel like your ready, then ask her out again.
