Talk to your doctor about seeing a dietician. Our doctor referred us to a dietician, and we got all kinds of advice on how to change his diet to help him gain more weight. Adding butter to everything, giving him ice cream, and other "more fattening" things, and even giving him pediasure once a day. As well as what foods to try to give him first, and which ones to wait until after to give him because they won't help as much.
