Quite the opposite I think - it would be rude if you didn't offer your hand these days. Just don't make it one of those wet fish handshakes - I can't stand that! If you are going to shake a girl's hand then make sure it's nice and firm - not too firm though - don't want to break it either. It's gotta be just right.

Absolutely not. It shows that you consider her an equal. I always shake the hand of a person, male or female, that I'm meeting for the first time. (Although I admit it tends to catch most people off-guard) It's still the right thing to do, though.

No, it's polite -- if you're looking her in the eye. Remember that politeness isn't set in stone; there is no Ten Commandments of Etiquette. The object is to show respect and interest for those around you, and humility towards.

it is fine -- just remember some women will shake like you do and some will shake sideways which is more femine especially for older women. it is best to give them a little room to lead as to which they prefer. and shaking with a woman isn't a squish contest like it is with a man -- firm but gentle.

no it's probably the best way to introduce yourself for both men and women.. it's showing you're interested in saying hello but not too over the edge.
