You need to have the rabies (which only a vet can give) and the DHLLP also know as the 5 way. If plan on boarding or if your dog will be around alot of other dogs, you may want to get a boardetella vaccine, but it isn't necessary. You can get this shot at you local feed store or Tractor Supply.
You can also get all of your dewormer here. (Neomex is a good brand, as well as champion. I don't use anything else)
When giving the shot, be sure there is no air left in the syringe. That is very important! There cant even be any in the liquid. Then read the directions on the back, basically you just pull up the extra skin on the back of the neck (NO MUSCLE!!!!!!!) poke and inject. If your dog is jumpy, you really need someone to hold and/or distract him. This could be letting him lick on a Popsicle, trying to lick peanut butter out a Kong or whatever
Good luck!!!

Puppies actually need several sets of vaccinations, starting when they're 6-8 weeks old. While it's obviously ideal to start their vaccines this young, it's never too late to go in. Take your puppy to the vet for a check-up, vaccinations, and de-worming. Since she's four months old, you'll want to schedule an appointment for her to be spayed, too.
Owning a dog is expensive. Vet care costs a lot, but it's well worth it. If you're not willing to care for your dog's medical needs, you shouldn't be owning a dog. Schedule a vet appointment for her as soon as you can.

I would not recommend that. Take her to a vet, they know what they are doing.
If you really don't have the money, I would just skip the shots. Some of them are not absolutely necessary, like the kennel cough vaccine.
