A post mortem is the gross examination of a body. The body will be checked for overt signs of abuse, neglect, torture, sexual assault. Somtimes organ systems will be examined; an abnormally enlarged organ could possibly be indicative of possible drugs or poisoning. Biopsies may be taken to check for possible drugs or poisoning agents such as arsenic, sedatives and the like. Pictures are often taken if there is suspected abnormality so the pathologist may examine and compare to other medical sources.
If there is no clues to cause of death in the post mortem, that is where the toxicology reports come in. The tissues, blood, other body fluids will be tested for drugs/poisons to determine cause of death.

basically they dont know why the person died; or perhaps there are several possible causes, and they cant work out which one proved fatal.
there are several 'levels' of post-mortem though, so sometimes if one proves inconclusive, others are run instead
