I am Buddhist, allow me to answer this from a Buddhist perspective to give you some small insight for comparison with the other fine answers already posited.
Self Realization in the Buddhist sense is to become self aware. This concept in Buddhism is very important in understanding the first of the Four Noble Truths and can be confusing to a novice, it is called the concept of "Not I". Let me explain...
We are made up of several things, each to a Buddhist is impermanent and ever changing. One of these things is consciousness. To Buddhists our consciousness is very changeable as there are 82 differing consciousness types.
To understand that our essence is completely seperate from our consciousness is to realise that we...as in our essence...are "Not I" as in our consciousness is to become aware of the impermanence and duality of self...we become self aware.
Peace my friend from a Buddhist in the continuing Christian season of joy.

To find the Real Self, your true essence. Yogananda founded a Self-Realization Fellowship that shows one way of doing this. Sufis have another way, as well.

It means you get to be everything you ever wanted to be, so you can be really not much good to anyone else becasue you are so much more important.
Unless you've always wanted to be a doormat, then you might actually be of value.
America has got to get away from the SELFishness already.

It is realizing that you are who you are. Realizing that you are complete. Most of us think there should be more to us and it makes them unhappy.

As it applies to self-concept, in the healthy sense, it is a concept of your qualities, both positive and negative.

To understand who you are, to come to terms with that person and to realize that that person is worthwhile and worth being.
