Keep a journal of your good days--those days when you accomplish everything you have set out to. Write about everything--the weather, the outdoor temperature, how you are feeling physically, and everything that has gone on.
Keep another journal of your bad days--those days that you don't get much accomplished. Also keep track of what kind of day it was, how you were feeling, and what else has gone on over the course of the day.
Read the "good" journal often--see what it is about those days that kept you going in the right direction.
Also, reward yourself on your very good days--you deserve it.
Don't forget to write about where your future will go if you stay focused and motivated. How great things can get if you stay on track.

Find a motivational icon. Mine is Henry Rollins, but whoever works for you. Imagine justifying your slacking to the icon and their sardonic response (Jeremy Paxman is good for this too...)
Then get back to the task in hand.
