(the others above are close to how I would say it, but this seems the most natural-sounding to me)

I wanted to let you know that my feelings for you have changed. I realize now what a cold, hypocritical person you are. Good thing I won't have to ever see you again, and by the way, you are not a professional doctor. And you're so dumb, it's not even funny.

I wanted to let you know that my feelings about you have changed. I now realize what a cold hypocrite person you are. Good job! Now I don't have to see you anymore and you are not a professional doctor. You are so dumb, it's not even funny!

I want to let you know that my feelings for you have changed. I finally realized what a cold and hypocritical person you are. You are not a professional doctor and I am glad I don't have to see you again. Furthermore, you are so dumb it isn't even funny.

I wanted to let you know that my feelings for you have changed.
I realize how much of a hypocritical cold person you are.
It's a good thing I don't have to see you again. By the way... you are not a professional doctor. (could substitute for: you are such a lousy doctor).
And you are so dumb it's not even funny.
