and stress, according to the dr. How can I improve his diet?

He is 15 and he is my step son who lives with me. I think he is stressing about his mom wh has just taken her abusive husband back. He is going there this weekend for the first time in a long time. I think that triggered the black-outs!? He always has stressed a lot he is just like his father.

If thats all the advise the doctor provided, thats pretty poor.
May i suggest you consider Anxiety.
Some individuals bodies are less resistant to common foods when frequently consumed. Such as;Soft drinks & energy, & pre mixed drinks - they contain high levels of caffine and preservatives (these act as a stimulant and can trigger anxiety) & all other foods that are procesed (not fresh)
Also, you may want to feed your son lean cuts of meat, including red meat, insted of processed meat from fast food joints & pre packed & made meals. Red meat provides high levels of protein and heomoeglobin which helps the body transport oxygen through the blood. A lack of this may cause anemia which is characterised by fatigue, lathergy and at extreams may encourage blackouts.
One other suggestion, is your sons blood sugar levels - diabetes?
Or High or low blood pressure.
Don't be alarmed these are not life treatening conditons and are easily addressed and fixed.I have at different points in my life experince black outs - and the above condtions.

The stress part may be what is causing the poor diet. Work on that part first. Get him some therapy to help him cope with the stress he is not dealing with in a healthy way. The eating thing will be affected by how he feels about himself. Get that straight first.The rest will come along with that.

Ensure that he is getting a balance diet of fruit vegetables, protein, fat and carbohydrates. It might be a good idea depending on his age and whether he thinks its a good idea that you make him take a pack lunch to work that way you know that you are helping him.

Always make sure that he has breakfast, even if its just a smoothie make sure that he eats before he leaves the house in the morning.

Make sure that you include lots of chicken and veg in his diet and maybe he should also consider taking vitamin tablets to help him as well

ensure his diet is rich in vitamins and minerals.

let him eat more food (healthy food)
veggies(even though not everytime), protein(meat etc).

and is it just due to poor diet and stress?
maybe he's got some illness or something.

How old is he?

How can you improve diet? well get him to eat every good food like lean chicken, tuna, turkey, sea food, good mix of carbs and other essential elements(vitamins, minerals etc).
