I had to cook a sausage so I hit the minute button on the microwave and when I opened the door a fly flew out. Hows that possible? I thought microwaves killed all bacteria and bugs.

With such a small mass of water (the moleucles that vibrate to make heat in micorwaves) and its own cooling system (wings) the rate of heat the microwave can put into the bug may be slower than the bug can disipate it. Try it pinned down with a toothpick and try it without wings. Max power for longer.

I presume it must stay towards the bottom of the encasing so to ensure the microwaves don't sweep him by. Is it possible that he was under the item you had placed in the oven, with such a small cavity to breathe through so that he could survive? Possible.

is it possible that it just appeared that the fly came out the microwave when you opened the door? That just doesnt' sound possible. Although while reading your question I couldn't help but think of the movie 'the fly'.
anyhow, maybe you are mistaken.

Eh.. Catch a fly and try it again. Perhaps with some research you'll find some new material for the next nobel prize :)

Seriously I have no idea. He should be fried like all other tiny ugly animals put into the micro.

The fly is too small to absorb appreciable microwave energy. It's much much shorter than a wavelength.
