Lots and LOTS of water are the only way to pass stones. If the doctors say he needs surgery to remove them, then that is what is necessary. If a stone becomes lodged and is too big to pass through the ureter (the tube connecting the kidneys and bladder), you will develop a hydronephrosis (water backing up in the kidney) and at that point you have bigger problems to worry about. Infection could set in is the biggest concern and kidney damage as a result of massive hydronephrosis.
You'll also probably want to get some narcotic pain medication, as kidney stones are extremely painful to pass.
In summary, he'll need to drink Tons of water to get them to pass, and when and if they do, he'll be free of pain.

If he can't afford pain medication also, can he afford any other supplements, herbs etc.? Is there no option for those without insurance? I live in India.

If he can avoid all diary products (milk, cheese, yoghurt etc.), tomatoes and spinach, and drinks lots of water, the stones may get washed off over time but that is hoping.
