Help-I filled out a form with my name,b-day,address,and social security # and lost the paper.What should I do?

I dost this information some time today in a public shopping center. I want to know what I can do to prevent something from happening to my identity.

To add to the great answer that the previous poster said,
Phone numbers for each of the credit bureaus are:

Equifax 1-800-525-6285
Experian 1-888-397-3742
TransUnion 1-800-680-7289

It is my understanding that if you call one they will get in touch with the rest of them for you!(ask about that to make sure)
tell them you want a FRAUD alert placed on your account

You can place a fraud warning on your credit with each of the three credit reporting bureaus.
