I have friends that meet this criteria and seems hard for most of them to find suitable mates due to their lifes situation.

A person with a big heart can look past a physical disability. Mental disability depends totally on the disability. Aside from that, I'd say in general only poor and fat women would settle for poor and fat men. It would help the man's chances if he developed some extraordinary qualities, like being a very witty conversationalist or a virtuoso musician.

I don't know, but you said nothing of the man's personality. I think that could make a world of difference.
I'm guessing it would probably not be love at first sight, but I'm sure love is possible for such a person, if somebody gets a chance to know them.
