I wouldn't argue with the rest of the answers so far, but I don;t think anything that was mentioned is really the cruelest way... here's what I think:
To drive your man absolutely nuts you need to get him a little boozed up (only a little), then you take him to your place (or his), make him sit on a chair and tie him to it. Tie his hands behind his back and his body to the chair.
Then you put on some really sexy music and start dancing for him... very seductively. Occasionally lean in close enough for him to be able to smell you! But remember, he can't touch you. So move your body infront of him... put your leg on his lap... sit on his lap with your back to him (all while dancing. Gently nibble on his ears and kiss his neck. Rub up against him occasionally. And then ofcourse, make sure you take a little something off every now and then... etc.
Hun that is creal, believe me.
One word of warning though... if you ever plan on doing this... make very very sure that you want to have sex afterwards... because by the time you untie your man - he's going to be a beast.

I wouldn't know from a personal perspective, as I hate playing games or hurting others. But I guess it would be to pretend you're interested in a man when you're not...lead him on. Or flirt with his friends and try to make him jealous. Have you ever seen "How to Lose a Guy in Ten Days?" Kate Hudson tries every trick in the book, just about, to torture her boyfriend.

making him think he's going to get laid at the end of the night, and you know damn well he's not. I think girls that do that are whores to be quite honest with you. It's humiliating to the guy and there really is such a thing as "blue balls". And from what I understand very painful.
