By using special rods and a solution that curls the hair. Perm means permanent wave I believe. The risks are that the hair may get burned if left on for too long- it will be damaged and dried. If you want to get it done, make sure to see a professional.

You go to a salon and the stylist will use a chemical solution to alter the chemical makeup of your hair. For a curly perm he will use rollers to set it, then wash it. For a straight perm (relaxer), he will apply the solution then blow dry it straight after washing. The risks are hair damage so if you have had a lot of processes (such as coloring) it is not recommended. Do coloring at a later date. "Perm" just means permanent. It is a permanent change to your hair's structure and must be grown out.

well you have to go to a salon and get it done there... a perm is like permanent straight or curly hair for about 5 months or more. if your hair is naturally straight now, and you want it to be curly, you can get it done curly and it will stay that way no matter what you do to it, it'll be like your natural hair. risks involved would be that it damages your hair a bit because of the chemicals they use.
hope i helped =)
