Officially it's the slogan of the state tourism commission. However, it is mostly used to refer to the city.

I Love NY was developed as an advertising slogan by the state for their tourism campaign quite a long time ago, it does refer to the entire state but truly is geared more towards the city itself. I lived on Long Island in NY for 40 years and never heard of anyone thinking of it as referring to our area.
