It is cramping because of overuse. Really, the only way to stop or prevent it is to take breaks every so often while your writing. This will relax your muscles and help them recover.

Start with 2 simple things - 1st put something around the writing implement (cause it to get BIGGER around) to make it so you can relax your hand more. 2nd add potassium and magnesium to your diet by eating a banana a day.
If you can see a massage therapist to help please do. If not I have more homework for you or your parents. Read The Trigger Point Therapy Workbook by Davies. It teaches about muscles, pain and referred pain and teaches you how to help yourself.

Could you possibly tape your notes? Or try a fatter pen, if your not gripping it as tightly as a thinner style it may help. Have you tries a pen with a comfort grip???

Put ice on it. Or if you don't like ice, put a warm washcloth on it and let it settle down. I have the same thing. Its called "Writer's Cramp"
