It's the silly plastic wrapping stuff that makes it sweaty and then it turns into mould. Annoying really because you could at least make like bruschettas or toast slightly stale bread but when it's mouldy you can't even give it to the dog!!!!

Bread is usually kept in the plastic bag that it is bought in. Moisture in this bag collects, and allows mold spores to grow. Since the air is pretty moist, the bread does not get stale. If you do not keep it in a bag, but do have you heat on, that could also create mold. Heat adds moisture to the air often, thus making mold form. Usually bread will go stale in "dry" air.

Because we have plastic to keep it wrapped in, which locks moisture in. Moisture causes mold. Lack of moisture causes staleness.
