Could they be under the influence of alcohol and/or drugs...

I suppose that's a possiblity, but I've always heard of that as a sign of head injury or concussion. That's one of the things docs look for when someone has hit their head.

If pupils are different size, it's usually from a previous injury, esp to the eye. They can be different if they have direct injury and sometimes the pupil never goes back to 'normal' and sometimes they can be different with brain trauma, and swelling in the brain. Usually drugs is going to make them both different, so my guess with your friend is that it's alwaiys been different, and it was just noticed. Either that or I guess it's possible the drugs themselves caused an internal problem with the eye, similar to a stroke.
