Can they deny or cancell your ability to have med. insurance if you don't get the physical annually? Including the requirement that family members also have annual physicals?

If they are self-insured company, then they can absolutely do that. They are not bound by ERISA, and they can apply whatever clauses they want.
A very large portion of medical claims come from undiagnosed issues, In addition, preventative care is much cheaper than care will be after the fact.

I never heard of it. Carriers generally can not cancel health or life insurance policies due to change of health after you have the policy unless they cancel everyone with that type of policy.
