This actually happened to me.
One morning you get out of your car and wait for the elevator in your office parking garage (very slow elevator can take 5mins), standing next to a 20ish-30ish boy you don't know. Just the two of you no one else around.
You notice his fly is down and it is obvious he is not wearing underwear. His manhood is not completely sticking out of his pants but it is clearly visable. He shows no signs of knowing his fly is down.
What do you do?

I will clear my throat loudly and ask politely if he was thinking of flying today. If he doesn't know what I mean, I would just gently look at him and go with my eyes down and point with my eyes.
He'll be forever grateful to me, maybe even in the elevator.
Just kidding......................................!!!!!!!!

If I was worried he was a perv, I would probably have pretended I forgot something in my car and gone back until he had stepped onto the elevator, then taken the next elevator or the stairs.
If it really looked like an accident and he didn't know, I would have waited until the elevator was just about to his floor and let him know the mouse was out of the house. If, of course, I could keep a straight face during the ride, which I sort of doubt...

The same as I would to someone who IS wearing underwear. "Excuse me, did you know your fly is open?"
Smile and pretend it's the most natural thing in the world to say something like that - which it is.
If you try to explain that you didn't intentionally LOOK at his fly, you'll just complicate the scenario. If you did, it's none of his business anyway.

How embarrassing! I would have waited until I was out of the elevator and said "excuse me, but your fly is open" and walked away, or I probably would have been incredibly embarrassed and not said anything.

Hm... embarrassing situation for both people, I'd imagine.
It would be great if I could figure out a way to discreetly tell him, but it's more difficult for a woman to tell this to a man.

OMG. First, WHY WERE YOU NOT WEARING UNDERWEAR???
Second, that is a toughie. I would probably turn red and decide to take the stairs because I wouldn't be able to not laugh and I would have trouble telling him his pecker is poking out!

I think he would guess something was wrong when I turned bright red! lol. Until it happens, I'm not sure how I would react. I would like to think I could tell him quietly that he had his fly undone. But that would embarrass him. I'm really not sure.

If I sense he's not mentally handicapped: I'd Sidle up to him and say "Hey baby, Hows everything hanging ? !
I'd accentuate on the word hanging whilst looking hard at his mishap to let him know.
If he be mentally handicapped Well theres nothing to be done and walk away politely.
