Fill in the 1st blank with whatever you have in mind to do with him in the future.
If he cooked for you or did something else special fill in the second blank with that. Hope this gets you started on where you need to go.

short and sweet, if you both like each other he will just be happy to hear from you and will give you a call. Or even say something in the e-mail like "I was thinking about going to see (some movie) are you free this weekend? maybe we could go see it"
Theres no need to go into too much detail or gush about how much you like him, you don't want to scare him off, just let him know you had a good time, he'll get the hint :-) especially if you say you want to hang out again.

I suggest a phone call. The more personal the better... I know its the age of technology, but actually PERSONAL communication will never get old. But call/email.... up to you... I would suggest saying something like this... "I had a great time over at your place doing (whatever it was). My turn to host next... why don't you come by (day/time) and we'll.... (eat dinner, rent movie... etc)"

how about something brief, it will get the message across without sounding a bit full on...something simple like "hey just emailing to catch up, I had a good tome the other day, hope I see you again soon! - keep in touch x"
