Fasting doesn't so much cause it, as your body's imbalanced Ph levels do (Ketosis). This causes yeast and fungi to grow well in your mouth. You can brush a lot, but really that's an uphill battle. The only way to cure it is to eat and restore your body's sugar/protein/fat intake, though really, if you're fasting long enough to cause bad breath, you've fasted too long. Anything longer than a day can hurt your body and cause muscle to start breaking down. Also, if you're doing it to loose weight, it will have the opposite effect and cause your body to store fat at an accelerated rate.
In other words, if you're fasting to the point of bad breath, stop immediately.

Yes, when food is not eaten, the body looks for other ways to find energy, such as drawing on glucose from the liver's stored glycogen and fatty acids from stored fat and eventually moving on to vital protein tissues. Once the glucose is significantly used up, the body's metabolism changes, producing ketone bodies. These cause sour-smelling breath. To prevent this, you should not go for hours on end without eating. If you absolutely have to fast (i.e. religious beliefs), then you should eat plenty before the fasting period or during the times you are allowed to eat normally. If you want to fast out of weight-loss reasons, make sure to eat little protein-laden snacks in between to keep your metabolism up (i.e. nuts or milk products). Don't forget to drink enough water, too.
Other options: chewing gum, eating mints, eating 0-calorie foods (cucumber, etc).
