Incline. Anyway, try to do both. Start your workout with flat and after about 10-15 minutes switch to incline. (Calories will only starts to burn after 15 minutes of each exercise). At the same time you should try to varies the speed from low to high and high to low. (Do not stay at the same speed at all times) On the last 15 minutes, go back to flat and do a slow jog and decrease the speed simutaneously for warm down. Keep a personal progress chart to see how much progress you have made. You'll get your goal better this way.

do it on an incline....but don't hang on as it will lead to sore back/hips/less work getting done
if you're going to walk at an incline you should never hang on to the railings and if you need to you're going to fast
