Remain healthy, physically and mentally. In both the cases you need to exert and take steps. If you are strong on both accounts, memory would multiply and your concentration would increase.
As far as myself is concerned I practise Yoga for the same. That gives me exercise for all body parts as well as mental strength which keeps me concentrated towards my assignments. Hope that can help you too. It is a slow process but a trusted one. Give it a go and see by yourself.

drink water often, dehydration can cause lack of memory and concentration. Try to stay off energy drinks and high sugar snacks. Getting a good amount of exercise each day is also helpful.
You can also play games that involve memory like "memory" and solve puzzles to improve your mental sharpness. It is best not to crowd the brain when you memorise something so it is best esp if you are studying to study things in chunks so that your brain has space to process it all. Learning an instrument even if you arent all that musical may help as this involves a great deal of concentration and memory. However, You can only train your memory to a limit though, its usually also about learning strategies so that you don't forget stuff, like writing things down, using visual cues, repeating things to yourself etc.

A well rounded diet: the more colors in your vegetable plate, the more vitamins you're getting. Take vitamin supplements.
Stress is known to cause memory impairment. A proven way to relieve stress is by having a regular workout routine, getting enough sleep (at least 6 hours if not 8).
In general, people dont consume enough water. Drink alot of it.
Also, meditation is getting broader acceptance in the west as a way to improve memory formation and concentration. Noone knows why, but it seems to work.
In summary, eat right, sleep right, stay active, and cut out colas /coffee and drink more water.
