A doctors job is to insure you get the best help possible. All he is doing is sending you to another doctor who has more experience in that particular field. He doesn't pay anything, nor does the specalist. That's why you get the bills!

No a doctor does not pay to send you to another specialist.
Most often there is no change of money at all A doctor might get a pat on the back aka some money for refering you to a specialist. I don't know what country you live in so i cant just say go find a different doctor to tend to you. I do suggest that your care is really up to you . Sure you cant refer yourself to a specialist but you can make sure you make it known you want that and then discuss why you have not been referred .
No patient should just trust that the doctor is right on top of there condition . We always need to consider how well we are being treated not just by the doctors but by nurses and all medical staff.
Well your doctor may be a wonderful doctor he may also be a doctor that just has too many patients in need of care at this time.
Don't blame him , just be aware that your not the only patient he has and as such you could fall though the cracks. Don't let that happen.

The doctor would not pay to send you to the neurologist, though sometimes the neurologist might pay the doctor a small fee to refer you to him specifically, in order to give him more business. This is called a "kickback" and is illegal -- but it does happen sometimes (interestingly though, lawyers do it all the time, and it's completely legal!).
I imagine your doctor might be taking a watchful-waiting approach to your problem, to see if it resolves on its own, as many minor neuronal issue do. But if it's really gotten much worse, then you need to express it to him, or go see another doctor.

The neurologist will cost YOU a lot. Neurologists are specialists....they got advanced training in neurological dysfunction- your general doctor did not. If you don't have insurance, you may find it very difficult to afford a neurologist...In my area a neurologist charges $600 per visit. Also, your doctor may be hesitant to refer you out to a neuro because that means he is getting less money for you. Since it is your health, tell him to give you a referral if that is what you think you need.

Medical referrals are at best a catch 22 for all concerned. HMO's and insurance plans place restrictions on referrals due to cost. A physician who refers too many patients may not get his/her affiliation renewed. On the other hand specialists are courting other docs to send them referrals, to the extent of offering fee splitting or kickbacks - both of which are frowned upon by state medical societies.

No, it will cost you, not him.
If the symptoms are getting worse, which you indicate, call in, demand to be seen again, or go to the ER. If your doctor won't see you, call another doctor and request an urgent appointment.
Your doctor is expecting you to get better, and has an interest in your health. If he knew your symptoms were getting worse, he would see you today.

No. Its some times the other way around. It's illegal, but it happens If your in an HMO, there is external pressure to prevent you from going to see a specialist. If you have a HMO you sometimes have to call them and fight for it.

YOU pay for every doctor you see. Some doctors do not like to refer patients to specialists because they want to handle the problem themselves, not because they actually pay for your appointments.
