All have come to pass except for the end time prophecies which begins in Chapter 20. Chapter 19 ended with the destruction of the pagan Roman empire and the start of the Christian era.

None of the them have come to pass yet.
They are a totally different set of prophecies to be fulfilled; and only when the Antichrist comes as the world leader will they start. Then there will be seven years for them to all be fulfilled.
They will start when the Restrainer/restrainer is removed. The Restrainer is the Holy Spirit/Comforter who came after Christ went to prepare a place for those who believe. The restrainer are those who are true believers .. the Holy Spirit resides in them.. therefore once the Restrainer is removed so shall all those who believe in Christ Jesus.

Hmmm, no I honestly can't think of anything. As far as I can remember about the book of Revelation, these are all things yet to come.
