You should know that, the character who tells this, speak about one of her friends who recently become very fat. Then she continue: "Six pounds if it's an ounce, and I only saw her last week. Six pounds in a week!"

To explain this sentence, I would have to describe some very complicated rules of English grammar. Basically, the sentance can be translated to:

"I thought she could not gain any more weight and then she did!"

The second sentence is even more complicated. To get a more accurate translation you would have to talk to a professional translator, but that sentence basically means:

"She has gained at least six pounds since I last saw her and I saw her last week. She has gained six pounds in a week!"

Without knowing the context it sounds like the person is referring to the sudden weight gain. She thinks the other has gained all the weight she possibly can and then all of of sudden she's gained at least another 6 pounds in only one week.
