the vatican and rome are major popular places. the vatican is its own place really, not so much a city. Ive heard the southern part is great.

Cinque Terre!!!!
Its five villiages all along the italian riviera. Very small villiages by the way. There is an 18mi trail that you can walk that takes you along the mountains and thru peoples vineyards. The place is amazing!!! I stayed there for three days. One of my favorite places in Italy.
Naples!! Head down there, and also visit Pompeii. Naples has the best pizza ever! Brandis is really good, the original pizza started there or the Margaretta pizza something like that. Plus, its very crazy out there, but amusing to say the least.
Amalfi Coast: Salerno area! Beautiful beaches here as well. Tons of small villiages where you can take a ferry or bus to each location. Worth a few days visit here. Plus some really cool caves you can go visit.
Certaldo: This is an hour out from Florence. You can visit Certaldo, along with Siena and San Gimiano. These are your small Tuscan cities that are filled with beauty and its not as touristy. Plus San Gimiano has a gelati place that was voted best gelati 2006. And boy did they earn it b/c that stuff was AMAZING!!
Florence is an amazing city. If you are into the arts, this is the place to come see Italian Renaissance at its finest.
Venice: Has that romantic touch. Everything is so close together, but you can get lost wandering the small streets of Venice, but you always end up find your way back. Plus the lighting there is amazing. The way the sun hits Venice Piazza is truely breathtaking. No wonder artist flocked to Venice to paint. The lighting is amazing.

Venice - just get a water bus pass and travel up the main canal from train station to San Marcos square. Be sure to stop and shop around the rialto bridge, to catch a couple churches along the way, and then to feed the birds at San Marcos. Also be sure to walk along from the square to see the bridge of sighs, and to catch the tower and the columns around the area of the square (and maybe a coffee at one of the restaurants). I'd spend two days in Venice, one to get lost shopping in back streets and maybe catch a fairy out to one of the nearby islands.
Pisa - the leaning tower is the most impressive, at 20 euros to climb it. Don't do it if you're afraid of heights, the leaning only makes the effect worse. Also, the baptistry and the cathedral are worth paying to see. There are several other churches in the city that are nice to visit, including a small gothic cathedral said to hold a thorn from Jesus' crown. Ultimately, the only big reason to go to Pisa is the leaning tower and surrounding areas. Takes about one day to feel satisfied with a visit to Pisa.
Florence - To see: midievil bridge, the statue of David (MUST SEE), and the baptistry doors (or, at least, the good replicas at the cathedral. You can be satisfied with one day of time in Florence.
Rome - the capitol... Too much to see, really, spend a whole week if you can. The vatican, the pantheon, santa maria in travastere, santa maria in aracoli, the tomb of the unknown, the catacombs, Trajans market/column, the trevi fountain, the colosseum, the roman forum, san giovanni in laterna, Santa Maria sopra Minerva, san pietro e paolo, san pietro in vincoli, the spanish steps, the triumphant arches, castel sant angelo, campo di fiori, piazza navona, La bocca de Veritas, etc, etc, etc
Capri - an island that used to be a retreat for roman empires, this island features all the best: perfect weather, vistas from the mountain, incredible landmasses, italian small village charm, shopping for items you can't find anywhere else (if you buy it on the island, it was made there), fantastic seafood, and a possible tour to a cave that is only lit by light bouncing off the sea floor and lighting the interior an intense blue (google for pictures, you'll see). A must see, I'd spend 2 days if you can find a hotel on the island.
Some cities of interest that I never did get to catch: Sicily, Milan, Geneva, Verona, Mantua, Naples (although I've heard naples is kind of dirty), and the old roman ruins of Pompeii (supposedly a MUST SEE)

Aside from Venice, Florence and Rome which every first time visitor to Italy should see, where you go depends on your interest. If you are interested in archaeology, head to Naples and visit the archaeologoy museum as well as Pompeii, Herculaneum and Paestum (south of Salerno). If romance is your thing go to Verona or the Amalfi Coast. If you like hiking and scenic views try the Dolomites or the Cinque Terre. If you traveling to Italy for the beach go to Sardinia. And if you want to see the best Greek (this is not a typo) ruins in Europe make your way to Sicily.

Well I am a beach Lover and seeing that Italy has some of the most beautiful beaches in the world I would check out the coastel areas like the Amalfi coast in Napoli inclusive of Capri and Ischia....Tropea is Sothern Calabria, Giardini Naxos/Taormina in Sicilia and last but certainly not least, Costa smeralda/Port Cervo in Sardegna, which is mainly a holiday area for Italians...Take it from someone who spent 3 months ever summer there since I was born, these are some of the most amazing places on earth...

La Maddelena, Sardegna is a very popular tourist spot. I lived there for a year. Great beaches, shopping, food, and the sites are amazing! The people are friendly. I plan to go back there again some day.
Rome is a great place to see architecture! The old ruins...trevie fountain, coleseum. and much more.
Google them for pics!

Go to Siginilla and eat real Italian Spaghetti and buy some Italian shoes. Meet an American Navy Man and live happily ever after... lol

Two top coastal area in Italy you won't regret visiting is the Amalfi Coast area and the Cinque Terra area. Amalfi Coast is by Napoli Cinque Terra is by Genoa
