How to order a pathology report?

here is my situation. My mom passed away 10 years ago from breast cancer and now I am part of a genetics study and they said they were going to send me the forms to send to the dr. I live in WA, she was in cali, and died in cali. Would the name of her dr be on her death certificate?

It could be but you can also check with the hospital she was receiving treatment at and get the reports and you should be able to get all the info you need. I'm not sure on the laws in CA in regards to pt who have passed on medical records but they could possibly still have everything. Good Luck!
