I love you = Te quiero
= Te amo

I love (Name) = Le quiero a (Nombre)
= Le amo a (Nombre)

Name, I love you = (Nombre), te quiero
= (Nombre), te amo

Note: I know that BOTH verbs are used to say "I love you" in Spain, and Martha P said that both are used in South America too. So I'd say you're safe choosing either one! In Spain, they usually use "querer" (te quiero) to say it. It's a bit more common and more informal. "Amar" (te amo) is usually used in a slightly more formal way, or to express a very intimate love. But both are acceptable.

Te quiero/te amo

Te quiero (name)

(name) te quiero

For all those ones that keep insisting. Te quiero ALSO means love, at least in South America.
