Headache and nausea are common symptoms of carbon monoxide poisoning.
A leak in the exhaust system can allow carbon monoxide into the passenger area!! Have the exhaust system checked today!!! Until then, anyone in the car should leave the windows down to allow fresh air in!!

There are a few more questions to ask about this to try to get ot the bottom of the problem. Eg. When is this usually happening, first thing in the morning? Has she eaten breakfast? If not that could make her nauseous. How long has it been happening for? Could she be pregnant? Does it begin before she even starts the car? Is it just her car or all cars? What if she is a passenger, does she get sick then? Any pets? Is the car operating properly? Maybe excess exhaust could be making her sick. I would see a doctor if this continues as it isn't normal and could be quite debilitating if she is unable to drive, as I'm sure you are aware. Hope you find an answer soon.

Like someone else said it could be a carbon monoxide leak. When she gets headache get some fresh air into that car. Even pull over and get some fresh air before something happens that is not fixable. If it continues have your mechanic check the car for leaks of exhaust gas line leaking.
Good Luck hope she gets better.

My best guess would be exaust is leaking into the car causing carbon monoxide poisoning. Other than being tired, she has all the symptoms. If it only happens in that car I'd have it looked at.

get a battery operated carbon monoxide detector and have her put it in the seat next to her, just to rule out a possible CO exhaust leak, other than that I don't know

you might want to have the car checked to see that it's not emitting carbon monoxide into the cab of the car...
