Dehydration is a risk among users who are physically active (such as dancing for an extended period of time at a night club or rave) and forget to drink adequate fluid, as the drug, being an amphetamine-based stimulant in addition to its empathogenic properties, may mask one's normal sense of exhaustion and thirst, and this risk is much greater when used in combination with alcohol. Alcohol and dancing heavily dehydrate you while the stimulating effects mask these signs and give you energy to continue far past normal capacities. Hypertension is another risk factor given the increased cardiac load experienced while under the effects. The effects of long-term use in humans are largely unknown and the subject of much controversy - particularly with regard to the risks of severe long-term depression as a result of a reduction in the natural production of serotonin. Excessive or heavy use would likely cause mild to moderate damage to the nervous system.
