I worked one day for a company last year; they take your pictures sitting in a vehicle, simulating an accident, and I received $150.00 (without tax) for it. I asked the agency that sent me to do this job, they will not send me W2 form because I did not make more tahn $600.00 with them. Do I need to declare this (150.00) as a part of my income? If so, what form should I use?

Basic rule of thumb, if you do not received a W-2 or 1099, you don't need to claim it. If however you feel the need, use a form 1040EZ and fill it in the Misc. Income section.

Yes, it's income. Claim it on Line 21 of Form 1040.

It should go on Line 21 of the 1040.
