I have to say that I have woken up in a lot of pain which didn't bother my sleep at all. My shoulder is killing me and I am quite sure I hurt it while I slept but didn't feel it. So I think that somehow it has someting to do with conscious thought.

i think you are confused between both the terms, anaesthesia and sleep are different. in anasthesia we make the pateint's pain senses numb so the pain receptors block and no pain perception is recieved at higher centres in brain and no pain is felt. but in normal sleep the person feels the pain.

Not quite true. One time I was having a dream and in it I felt stomach cramps. I woke up and I 'did' have cramps, the same with a headache.
