The black spot on his teeth is rotten decay... thats the cause of the bad breath. He should get that fixed.. before it gets into more serious problems.

If he has a cavity he needs to get it fixed at the dentist office. I doubt the cavity is the cause of his bad breath. He does need to brush often though and use a good mouthwash. Why not buy some listerine breath strips and offer him one now and then. If the bad breath is severe he should see a doc to get this checked out and see his dentist. He definately needs to get that cavity filled though.

I trust others - His well being care specialist is the guy to ask. For myself - I brush when I awaken and after each and each meal. i detect that cultured yogurt once an afternoon also takes my undesirable breath away and also dill or sweet pickles look to sparkling my tongue. I also use Act Mouthwash and bite Freedent Gum. Many solid solutions on your question. DeeJay

Cavities will cause bad breath. His dentist can fix that for him.
However, if he doesn't floss at all, that is the main culprit. When you floss, you pull out food that is sitting in between your teeth. The longer it sits, it starts to rot and stink.

Oh yeah, that could cause horrible breath. Basically, his tooth is rotting, and rotting anything stinks. Give him some mints, but he really needs to get that taken care of.
