Okay personal history means that the individual has had colon polyps. A colonoscopy is simply a procedure used to check and diagnose if the patient does currently have them or not. A check-up if you will.

Personal history of colon polyps means that they have polyps. Family history of polyps means that a first-degree relative (usually a parent) has polyps, and so they need to be screened for it as well.
