Your friend needs to see a Doctor straight away as at her age this is not a normal thing to be happening

Obviously,. she needs to see a doctor, who will check for any medical reasons for this. If none are found, then she will probably be referred to a psychiatrist for further evaluation.
There should be no delay in talking to a doctor.

I would talk to a doctor.
Also, she can wear diapers. As bad as that sounds, it will prevent messiness.
