I realised that my colleagues shout when they have earphones on (with music playing) and they do not seem to realise that they are shouting.

The same has been said of me - does someone have a medical explanation of this?

Sure ,, the same reason that many deaf people have difficulty talking, even though there is nothing wrong with their ability to create speech. We need to hear in order to speak properly. If we have music blasting in our ears, just like at a party, we tend to talk louder to be heard. They are trying to hear themselves talk, so they must talk louder. The competition of sound in the head makes this happen.

When we hear ourselves talk normally, we hear the sound both through the bones in our head and our ears. When people wear earphones, they are removing most of the ear portion, which only leaves the muffled sound that is conducted through our bones. Try plugging your ears and talking, you'll see what I am trying to get at here.
