what form do i use to file my 1099 earnings and w2 earnings together?

You will have to use form 1040 to report 1099 earnings. You are unable to use 1040EZ due to the earnings.

Actually, you must use 3 forms. Form 1040 -- the basic tax return, Schedule C or Schedule C-EZ to account for the 1099 income and any business expenses against it, and Schedule SE to calculate the Self-Employment tax (only if the 1099 income was more than $400).

The W2 income goes directly on the Form 1040 and the totals from the 2 schedules are transferred to the 1040 as well.
